ABSTRACT

BACKGROUND: The posterior anatomical locking plate has obtained excellent fixing performance in the treatment of complex fractures of the distal tibia; however, there are many drawbacks such as nail and plate broken, and the related rules are never reported. OBJECTIVE: To establish a finite element model of posterior anatomical locking plate for distal tibia and to provide references for its design and improvement. METHODS: The imaging data of a male patient aged 34 years old (body mass: 68 kg) with fracture on the left distal tibia were imported into Mimics16.01 software, and the proposed reconstruction parts were determined based on the default threshold of the software. The files were imported into the Ansys11.0 software, the distal tibial surface was fixed, and a force of 340 N was vertically loaded on the supine surface of the horizontal section of the large tibial shaft. RESULTS AND CONCLUSION: There was completely connected with the stepped surface of nut and screw thread portion in locking steel plate. The strain on the plate depended on the stress values. The posterior anatomical locking plate for distal tibia was more favorable for the fixation at the fracture site and could reduce the fixed plate and screws loosening, both of which could affect the fixed efficacy.

ABSTRACT

BACKGROUND: Percutaneous kyphoplasty is an effective method for the treatment of osteoporotic vertebral compression fracture, because it can relieve pain quickly, restore the vertebral height and correct kyphosis. However, little is reported on percutaneous kyphoplasty for the treatment of Kummell disease. OBJECTIVE: To evaluate the efficacy and safety of kyphoplasty with polymethylmethacrylate bone cement in treating Kummell disease in the elderly. METHODS: Thirty-one patients (6 males and 25 females) with Kummell disease, aged 58-72 years old, were included in this study. Affected segments involved T11 in 5 cases, T12 in 11 cases, L1 in 8 cases, L2 in 4 cases, L3 in 2 cases, L4 in 1 case. All the patients underwent percutaneous kyphoplasty with polymethylmethacrylate bone cement. The clinical effect was evaluated by pain, viability and imaging changes before and after therapy. RESULTS AND CONCLUSION: (1) Pain relief: The visual analogue scale score for all the 31 patients at 3 days, 3 months and 12 months after the surgery were significantly lowered compared to the preoperative scores (P < 0.05). (2) Viability: The mean Oswestry disability index scores for 31 patients at 3 days, 3 months and 12 months after the surgery were significantly lowered compared to the preoperative scores (P < 0.05). (3) Imaging evaluation: The anterior vertebral height and vertebral height at middle line for 31 patients at 3 days, 3 months and 12 months after the surgery were higher compared to the preoperative data (P < 0.05), and Cobb angles were significantly lowered compared to the preoperative data (P < 0.05). (4) Adverse reaction: There was no leakage of bone cement in the vertebral canal and vein, and no adverse reaction occurred related to bone cement. In summary, the kyphoplasty with polymethylmethacrylate bone cement is safe and effective in treating Kummell disease in the elderly. It can fast relieve pain, improve patients quality of life, and reconstruct the spinal stability.

ABSTRACT

OBJECTIVETo study the expression of β-catenin protein and the status of loss of heterozygosity (LOH) on chromsome 10q in medulloblastoma, with clinical correlation.

METHODSImmunohistochemical study for β-catenin protein was carried out in 50 cases of medulloblastoma encountered in the First Affiliated Hospital of Xinjiang Medical University during the period from 2002 to 2011, including 32 cases of classic medulloblastoma, 13 cases of desmoplastic medulloblastoma and 5 cases of medulloblastoma with extensive nodularity. The status of LOH on 10q was also detected by fluorescence in-situ hybridization. The clinicopathologic characteristics and prognostic parameters were studied by Kaplan-Meien and Cox analysis.

RESULTSThe rates of expression of β-catenin protein in classic medulloblastoma, desmoplastic medulloblastoma and medulloblastoma with extensive nodularity were 53.1% (17/32), 4/13 and 1/5, respectively. The rate of LOH on 10q was 33.3% (8/24) in classic medulloblastoma and 2/11 in desmoplastic medulloblastoma. There was no statistically significant difference between the two tumor types. Univariate analysis showed that the expression of β-catenin protein (P = 0.022), lack of LOH on 10q (P = 0.020), extensiveness of tumor resection (P < 0.01), radiotherapy (P = 0.002) and chemotherapy (P < 0.01) represented important prognostic factors.

CONCLUSIONSMedulloblastoma with expression of β-catenin protein and without LOH on 10q carries a better prognosis. Assessment of these parameters is helpful in evaluating prognosis and subsequent patient management.
